Liver Transplantation: Preoperative Evaluation with Multislice Computed Tomography

Liver transplantation,in chronic liver disease and liver failure, is a successful treatment option which increase 1 -year survival over 80%( 1 ). Today due to lack of cadaver, liver transplants are performed more often from live adult donor. However, live adult donor liver transplantation require a thorough examination preoperatively due to complex variations in hepatic vascular system. Multidetector computed tomography angiography (MDCTA) is a successful method in the preoperative evaluation of donor and recipient with the ability to show the variations in vascular system structure that can complicate surgical procedures or contraindication to transplantation. CT can provide necessary information about caudate lobe size, exclusion of the hepatocellular carcinoma or other malignancy, the venous system structure, perihepatic varices presence, celiac artery structure, exclusion of splenic artery aneurysm and iatrogenic venous shunt location of the receiver. In live donor candidates CT is a useful method with the ability to show, hepatic artery anatomy, source of segment IV artery, the hepatic venous anatomy, accessory hepatic veins and detection of the portal vein variation, as well as evaluation of the bile ducts ,liver volume and steatosis.
